Social Worker I Contract of Service  

• Conducts interview to all patients seeking admission/treatment to determine eligibility for MSS assistance.
• Provides outright counseling with patients and relatives before discharge patient in the hospital.
• Prepares referral letters and other communication necessary for networking, linkages and coordination with community resources.
• Attends and facilitates patient’s medical needs as well as the complaints.
• Regular recordings of progress notes/reports of abandoned/referred patients.
• Performs other duties that may be assigned from time to Time.

Qualifications:

• Must be a Registered Social Worker
• With valid license

Salary: Php 24, 816.00

Status of employment: Contract of Service
