- Conducts interview to all patients seeking admission/treatment to determine eligibility for MSS assistance.
- Provides outright counseling with patients and relatives before discharge patient in the hospital.
- Prepares referral letters and other communication necessary for networking, linkages and coordination with community resources.
- Attends and facilitates patient’s medical needs as well as the complaints.
- Regular recordings of progress notes/reports of abandoned/referred patients.
- Performs other duties that may be assigned from time to Time.
Kualifikasi Minimum
Qualifications:
- Must be a Registered Social Worker
- With valid license
Salary: Php 24, 816.00
Status of employment: Contract of Service
